Registered Practical Nurse (Inter Professional Care Team)

1 FTE

One of a network of provincial community health centres, the NorWest Community Health Centres is a community based, non‑profit organization funded by Ontario Health, emphasizing access to primary health care and prevention programs and services.

We are currently seeking a 1 FTE Registered Practical Nurse, as a valuable member of the Community Health Centres’ team who will be an effective, collaborative member of NorWest CHC providing clinical services to clients who face barriers to accessing care (under housed, living with mental health challenges or diagnoses, current or past substance use, complex health histories, 2SLGBTQIA+, newcomers, non‑insured). This is a mobile outreach position that will provide clinical care, illness prevention and health promotion in social housing units and other designated areas while collaborating with external partners.

The position has a competitive salary, an excellent group health benefit package and the Health Care of Ontario Pension Plan.

QUALIFICATIONS
  • RPN with current College of Nurses of Ontario registration.
  • 2-3 years experience as a Registered Practical Nurse.
  • Three to five years’ work experience in a combination of remote rural, hospital and community settings and/or public health along with experience as a Registered Practical Nurse providing care to individuals who face multiple barriers to care.
  • Awareness and sensitivity to socio-economic and cultural needs.
  • Awareness of and sensitivity to health issues particularly within populations who face barriers to accessing care (under housed, living with mental health challenges or diagnoses, current or past substance use, complex health histories, 2SLGBTQIA+, newcomers, non‑insured).
  • Demonstrated commitment to and knowledge of community-based health care.
  • Flexibility in work hours is required including evenings and weekends.
  • Awareness of northern community health care needs.
  • Valid driver’s license, access to a vehicle and ability to travel to other service locations.
