Qualifications

  • A valid driver’s license and vehicle as regular travel is required.

  • Develop and maintain an effective Care Plan.
  • Communicate and liaise with patients and interdisciplinary care team.
  • Provide exceptional nursing services in various settings.

Education

This is a Full-Time position for

Hours of work are 8 AM – 8 PM within a 2-week rotation inclusive of weekends, weekdays, and on-call work.

• Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
• Current College of Nurses of Ontario (CNO) registration in good standing

• Developing and maintaining an effective Care Plan with respect to Community Health Services, independently or in cooperation with the Ontario Health atHome.
• Working with patients to provide exceptional nursing services
• Providing services in a variety of settings which may include in-home, clinic, retirement home or school
• Communicating and liaising with patients, family members and others within the interdisciplinary care team
• Performing other clinical duties as required
