A leading healthcare organization in Canada is seeking a Registered Nurse to join its Palliative Care team. The successful candidate will provide care and support to improve the quality of life for individuals living with life-limiting illnesses. Responsibilities include nursing assessments, directing client care, and collaborating with healthcare teams to ensure high-quality, evidence-based patient care. Edmonton Zone Continuing Care provides services through five main program areas: Home Living, Supportive Living, Facility Living, Transition Services and Palliative Care. We are looking for a Registered Nurse to join our Palliative Care team to provide care and support to aid in improving the quality of living and End of Life for a person and/or family living with a life limiting illness. As a self-directed team member with Continuing Care in the Edmonton zone, the Registered Nurse works collaboratively with the health care team and has responsibilities, including assessment of needs, delivering appropriate therapeutic nursing services within professional practice standards, coordinating resources, establishing goals for client delivery, and evaluating outcomes. In addition, this position provides case management as well as direct client care that meets established clinical standards, and is evidence based & client centered. In collaboration with family & other partners, the Registered Nurse acts as a consultant on the team providing clinical expertise and direction to other team members regarding client cases, developing a care plan, based on those unmet needs, using the appropriate internal and external resources. As a member of the multidisciplinary team, the Registered Nurse is responsible for participating in staff development. This position facilitates complex discharges from Acute Care to the Home & works collaboratively with Acute Care Transition Coordinators and multidisciplinary teams within Continuing Care and develops recommendations to support client flow to Home Living options.
As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit holders will be paid $39.58 - $54.16 per hour. Upon obtaining a full licence, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii) as applicable.
Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification. Three years of recent (obtained in the last year) formal community Case Management experience. 2 years of recent nursing experience (obtained within the last 5 years) or completion of a refresher program within 2 years. Self-directed, ability to work independently & use sound professional judgement. Excellent organizational skills able to prioritize & efficiently organize many details in a timely manner. Communication and assessment skills to work cooperatively & effectively with healthcare team members, clients, & their families in a fast-paced environment. You may be required to demonstrate your computer knowledge and proficiency by completing an assessment. You must demonstrate sufficient typing speed to meet workload demand & navigate through various computer programs including Microsoft Office (Word, Excel, PowerPoint), Outlook & Internet. Physically capable of performing nursing tasks which could include transfer of clients. The successful candidate will work independently in clients’ homes, which could include congregate settings, single family dwellings, condos and/or walk-up apartments. The successful candidate must be able to work in different settings and environments, as well as be able to lift and carry medical bags, which weigh approximately 25-30 lbs. Ability to travel throughout the AHS Edmonton Zone in all-weather circumstances during the workday from network office, acute care, and community setting.
Baccalaureate degree in nursing preferred; skills and experience relevant to nursing practice in community sector will be considered. Palliative care experience within the last three years (community setting is preferred).
