Position Summary

Registered Nurse, Reactivation Care Centre Unit 2 & 3

Your goal in this position is to provide excellent patient and family driven care, in a supportive and culturally sensitive manner. You will do this by collaborating with an interdisciplinary team of care providers, with an emphasis on the assessment, education, prevention, maintenance, and restoration of patient health. North York General Hospital’s Reactivation Care Centre (RCC) is an interprofessional, collaborative care model designed for patients who no longer require hospital care but need restorative care to support their transition home or to another setting, such as long-term care.

North York General’s RCC is located at Finch Site (formerly Branson) at 555 Finch Ave West, a newly revitalized space for patients, caregivers, staff & physicians.

Qualifications
  • Must obtain and provide a current Police Vulnerable Sector Check.
  • Diploma or Baccalaureate Degree in Nursing.
  • Registered and in good standing with the College of Nurses of Ontario.
  • Current Basic Cardiac Life Support (BCLS) certification.
  • Minimum 2 years of recent medical/surgical or clinical nursing experience working with older adults, dementia and cognitively impaired populations preferred.
  • Recent health assessment course.
  • CNA certification in Geriatrics an asset.
  • Knowledge of orthopedics or rehabilitation nursing and/or rehabilitation nursing CNA certification preferred.
  • Bilingualism in French/English an asset.

This Part Time Temporary position is for 12 months with 8 & 12 hour shifts, days, nights and weekend coverage in the Reactivation Care Centre Unit 2 and 3 at 555 Finch Ave W. Compensation includes a competitive hourly rate, vacation pay and the opportunity to join the Hospital Pension Plan.
