Are you a passionate and skilled registered nurse (RN) specializing in wound, ostomy, and continence, looking to make a meaningful impact? If so, we want you to join our innovative VON healthcare team as an NSWOC!

Your Role:

As an NSWOC with VON, your specialized skills and knowledge will be leveraged to provide comprehensive wound assessments, ostomy care, and continence management. You may work in the client’s home or a clinic and will collaborate closely with an interdisciplinary team to create personalized care plans that promote healing, comfort, and improved quality of life for our clients. You will help alleviate healthcare system challenges through the NSWOC Power of Three; provision of higher quality care, reduced costs, and improved outcomes for patients.

Key Responsibilities:
  • Perform a detailed holistic assessment of clients with wound, ostomy, and/or continence related concerns.
  • Formulate treatment recommendations in collaboration with the interprofessional team, and develop a plan of care based on the holistic assessment of clients who require wound, ostomy, and/or continence care.
  • Provide education and mentorship experiences to build wound, ostomy and continence knowledge within VON.
  • Provide consultation to care providers within the organization.
  • Collaborate with the interprofessional team across the continuum of care.
  • Share clinical knowledge relating to wound, ostomy, and continence care to enable clients, families, and communities to make informed choices in determining health and service needs.
  • Remain updated on current evidence related to wound, ostomy, and continence best practices.
  • Provide service consistent with the standards established by VON and provincial regulatory bodies.
  • Describe and document nursing processes clearly, concisely, accurately, and appropriately, according to VON standards.
  • Describe and document client and family responses to care plan interventions using established procedures.
  • Promote the involvement of the client and family in determining health and service needs.
  • Negotiate with the client or family as necessary to ensure the effective delivery of care while respecting client choices.
  • Document rationale for referrals, changes in visits, and for discharge based on relevant, sound data and established guidelines.
  • Refer clients to appropriate categories of nursing providers, support providers, and other disciplines using established guidelines.
  • Identify and utilize tools and resources for relevant work and manage these resources.
  • Promote client‑directed care, self‑care, and independence through teaching and health promotion.
  • Utilize adult learning principles and established teaching materials to provide health teaching to clients and families.
  • Influence policy review and development in health care and nursing practice standards through involvement in VON, regulatory, professional organizations, and community organizations.
  • Practice in accordance with the quality and risk management framework of VON Canada and Occupational Health and Safety legislation.
  • Participate in the creation of client care plans and follow up to promote continuous quality improvement.
  • Other duties as assigned.
Common Responsibilities:
  • Promote the goals and values of VON and its role as an integrated community care provider.
  • Promote a safe and healthy workplace ensuring conduct and activities are in accordance with the provincial Occupational Health and Safety Act and Regulations and compliant with the VON Safety Management System, including all policies, safe work practices and procedures.
  • Abide by all VON policies and work practices.
  • Abide by all confidentiality and protection of personal information policies, regulations and practices and ensure appropriate safeguards are in place within the role.
  • Work collaboratively with other staff in a team approach to service delivery.
External and Internal Relationships:
  • Engage regularly with clients, their families, and caregivers. Provide education and support to engage clients and families in independence.
  • Participate actively with other healthcare professional organizations and the community to promote nursing best practices.
Education, Designations and Experience:
  • Must be licensed as a Registered Nurse in the province of practice.
  • Completion of Nurses Specialized in Wound, Ostomy & Continence Canada (NSWOCC) WOC‑EP program, OR
  • International Interprofessional Wound Care Course (IIWCC), OR
    • Master’s prepared Advanced Practice Nurse (APN) with formal education in skin and wound care.
  • Three years of experience in the community or acute care setting with a focus on wound care is preferred.
  • Experience in community and home care is preferred.

  • A current and original copy of a satisfactory Criminal Records Check.
  • Must have a personal vehicle and possess both a current driver’s license and proof of vehicle insurance.
  • A Vulnerable Sector Search and/or a Child Abuse Registry Check may be required.
  • Available to work flexible hours.
  • Must be able to wear Personal Protective Equipment (PPE).
Working Conditions and Physical Demands:

This role requires a detail‑oriented approach in a dynamic environment, with physical activity including lifting, carrying (using proper techniques), bending, reaching, kneeling, and other movements that emphasize good body mechanics. Individuals in the role are required to walk, sit, stand, and climb stairs throughout the day, with some tasks requiring fine hand movements.
