Job summary

The Registered Nurse supports the client’s goals by assessing, planning, coordinating, and delivering home health care services for clients in Ashcroft, BC. You will develop and adapt care plans, provide direct care, assess eligibility, and collaborate with other health professionals to navigate community resources.

This is a Permanent Part Time (0.84) position with rotating day shifts from 08:30 to 16:30, based at Ashcroft Hospital and Health Centre.

Qualifications

• Licensed RN in British Columbia.
• Experience in home health care.
• Ability to work rotating day shifts.

Responsibilities

• Assess, plan, coordinate and deliver home health nursing care.
• Develop and adapt client care plans to meet goals.
• Provide direct nursing care and assess service eligibility.
• Collaborate with other health care providers and participate in committees/research as assigned.

About The Job

The Registered Nurse supports the client’s achievement of safe, realistic, and reasonable goals by assessing, planning, coordinating, organizing, implementing, evaluating and transitioning nursing and other care services for the client. The role develops, implements and adapts a care plan, delivers direct care, assesses service eligibility and provides care management to help clients manage their own care and navigate through the various services available within the community setting. Enhances quality of life for the client and family; consults, confers, and collaborates with other health care providers. Demonstrates continuous improvement and evidence based practice in nursing practice. Maintains and advances own clinical competence. Participates on designated committees/teams and approved research projects as assigned.

This is a Permanent Part Time (0.84) opportunity.

Shifts of work will be rotating days from 08:30 to 16:30.
