Job summary

Extendicare is seeking a part-time Registered Nurse to provide direct resident care, medication administration and care documentation in our long-term care setting in Ajax, Ontario.

You will collaborate with the Director of Health and Wellness and Director of Care, coordinate care plans, supervise unregulated providers, and respond to emergencies while upholding Extendicare's standards and values.

Qualifications

• Current registration as a Registered Nurse in applicable Province/State.
• Experience working with seniors and unregulated care providers.
• Education and experience in gerontology, rehabilitation, community/complex/long term care an asset.
• Strong written and oral communication, critical thinking, care coordination, time management and customer focus skills.
• Demonstrated ability to delegate to unregulated care providers.
• Maintain competencies per governing College standards.
• Proficient with computer and Microsoft Office applications.
• Current VPS in the last 6 months.

Responsibilities

• Complete and ensure residents’ care assessments and service plans are up to date.
• Assist with scheduling care conferences and participate in them.
• Work with DHW/DOC to facilitate moves in and transfers.
• Order, safeguard drugs/narcotics and administer per regulations and policy.
• Facilitate and provide resident/family education.
• Supervise resident self-medication assessments and audits.
• Supervise and delegate care and medication to unregulated care providers.
• Plan and evaluate care plans with the care team and substitute decision maker.
• Coordinate nursing team assignments for timely service delivery.
• Respond to emergencies and significant changes in residents’ condition.
• Coordinate infection prevention, health and safety and clinical programs.
• Coordinate scheduling of nursing students from approved institutions.
• Uphold Extendicare values and Code of Conduct.
• Complete other tasks as assigned.
