Job description

Within a multidisciplinary team the Nurse will coordinate early identification / screening, diagnostic, and intervention services for individuals experiencing first episode psychosis. The Nurse, in collaboration with the clinical team, will ensure the development and implementation of a comprehensive individualized treatment plan that mitigates the effects of a first episode of psychosis.

Responsibilities
  • Conduct initial assessment interviews.
  • Administer structured clinical scales.
  • In collaboration with the multidisciplinary team, the client and their families develop treatment plans based on client goals and knowledge of psychiatric and psychosocial treatment modalities.
  • Use recovery and rehabilitation models in assessment, planning, intervention, and evaluation of client and family‑centred interventions.
  • Engage in shared‑care with the client and the client’s primary practitioner.
  • Provide education to clients, families, and community partners.
  • Administration and monitoring of psychiatric medications including both oral and intramuscular injections.
  • Liaise and advocate between the client, family, other members of the treatment team, and the larger treatment community.
  • Provide crisis intervention and support.
  • Provide care acknowledging the unique needs of young adult clients and their families.
  • Support clients and/or families by meeting in both the office and community locations, including the client’s home and public meeting places.
  • Facilitate, with the client/family’s consent, the involvement of significant others / social network in the assessment and goal definition process.
  • Participate in team regional and/or community‑based group planning, implementation and evaluation as needed.
  • Participate in program evaluation, research, and best‑practice activities.
  • Attend team administrative/regional and other relevant system‑level meetings.
  • Complete required documentation and statistical collection in a timely manner.
  • Supervise students / volunteers as required.
Qualifications
  • Current registration and in good standing with the College of Nurses of Ontario required.
  • Current BCLS certification required at time of hire.
  • Knowledge, skills, and experience in the principles and practices of psychosocial rehabilitation and recovery including hope and empowerment.
  • Ability to work in collaborative partnership with clients, family, and community resources.
  • Ability to advocate on behalf of clients and families.
  • Working knowledge of the Ontario Mental Health Act.
  • Knowledge of community resources to support clients and families.
  • Skills in Microsoft Outlook, Windows Word, Excel, and PowerPoint; familiarity with accessing online laboratory results.
  • Knowledge and experience working with clients with concurrent disorders.
  • Ability or willingness to learn the administration of structured clinical scales.
  • Willingness to participate in research and program evaluation.
  • Evidence of continued professional development and education.
  • Valid driver’s licence and access to reliable transportation required.
Additional Information

Hours: Monday to Friday days including Mon Wed Thu Fri 8:30‑4:30 and Tuesday 11:00‑7:00 (Hours subject to change based on operational needs)

Site: Milton District Hospital, Georgetown Hospital, Acton Oakville Hospital

Salary: $40.24 to $57.68 per hour
