Job summary

Providence Health Care is seeking an HD Nurse to deliver patient-centred dialysis care within an interdisciplinary team. You will assess needs, build and evaluate care plans, and educate patients and families while upholding PHC's mission and values.

The role emphasizes teaching, collaboration, and leadership in a state-of-the-art health campus scheduled to open in 2027. Canadian RN in good standing with BC registration eligibility is required.

Qualifications

Summary

As an HD Nurse, you'll be at the forefront of patient care, working collaboratively within an interdisciplinary team to ensure patients/residents receive comprehensive support. Your responsibilities will include:

Minimum Nurse-to-Patient Ratio

Minimum Nurse-to-Patient Ratios (mNPR) set the minimum number of nurses required to safely care for patients on a given unit. Developed in partnership with the Ministry of Health, the BC Nurses' Union (BCNU), and health organizations across British Columbia, mNPR provides a consistent and transparent approach to staffing that supports safe nursing practice and high-quality patient care. What You Bring

Education:

  • You must be a registered nurse in Canada in good standing and be eligible for registration with the BC College of Nurses and Midwives (BCCNM).

Your Day to Day
  • Hemodialysis RNs care for patients with kidney failure undergoing dialysis treatment
  • Focus is on ensuring patients receive safe, effective, and compassionate care throughout their treatment.

  • Conduct thorough patient/resident assessments through chart reviews, observations, interviews, and collaboration with peers.
  • Develop, implement, and evaluate nursing care plans based on evidence-based practices, prioritizing patient/resident needs.
  • Adhere to established policies, procedures, and PHC standards while providing high-quality nursing care.
  • Monitor patient/resident progress, evaluate outcomes, and adjust care plans as needed for optimal results.
  • Maintain accurate records of assessments, care plans, patient/resident status, and discharge plans using manual and computerized systems.
  • Identify and address patient/resident and family learning needs, providing education and referrals as necessary.
  • Provide direction to staff, promote teamwork, and share clinical knowledge to enhance patient care.
  • Support nursing research initiatives and contribute to the improvement of patient/resident care policies and procedures.
  • Advocate for patients/residents' rights and dignity, facilitate discharge plans, and collaborate with families and community resources.
  • Perform additional duties as assigned to support the healthcare team.
