Job description
Purpose Statement

The GEM Nurse provides a specialized clinical resource for assessing and treating the frail elderly with a focus on frail seniors who are at risk of suffering adverse events, loss of independence and admission to hospital or long‑term care facilities. The GEM nurse provides expert and targeted assessment and treatment services; enhances linkages with hospital and community services and advocates for the frail seniors.

Duties
  • The GEM nurse is a geriatrics educator and capacity builder within the Emergency Department, the hospital and the community, to ensure that frail seniors at risk receive the right care, in the right place, at the right time.
  • To facilitate timely discharge of elderly patients from the ED and to link them to community resources.
  • To promote a model of best practice in Elder Care nursing and an elderly friendly philosophy of care within the ED.
  • Effective use of the Standards of Nursing Practice and the Nursing Process in assessing, planning, implementing, and evaluating the care of the patient.
  • Provides assistance in planning corrective actions to ensure safety for patients and staff.
  • Other duties as assigned.
Qualifications
  • Certificate of Competence from the College of Nurses of Ontario
  • Geriatric Resource Nurse Certification or other geriatric education
Experience
  • Minimum of 5 years recent (within 2 years) working in Geriatric Emergency Management.
  • Minimum of 5 years recent (within 2 years) completing assessments of geriatric patients.
Skills
  • Knowledge of the needs of frail elderly adults.
  • Demonstrated effectiveness working with minimal supervision with proven reliability and trustworthiness
  • Demonstrated adaptability and flexibility to respond to emerging and dynamic program needs
  • Demonstrated decision‑making skills assessing a comprehensive range of variables to interpret, analyse, solve problems and think quickly during crisis situations
  • Working knowledge of the psychological, physical and functional implications of the aging process.
  • Demonstrated ability to assess cognitive functioning and mental status in the elderly.
  • Knowledge in PIECES, GPA, U‑First!, and Montessori practices
  • Extensive knowledge of community resources.
Ability
  • Proven ability to liaise with referral sources, community agencies and other team members to enhance positive patient outcomes.
  • Excellent organizational, written and verbal communication skills.
  • Ability and experience working with Meditech and Microsoft Office programs.
  • Proven ability to maintain and promote positive interprofessional team relationships.
  • Demonstrated behaviour consistent with the hospital’s mission statement and core values.
  • Is compliant with legislation, organizational and departmental, regulations, standards and procedures related to occupational health and safety.
