The Community Health Registered Nurse, as part of the Primary Care team, is expected to work in an interdisciplinary team to deliver high-quality primary health care services for optimal individual and community health outcomes. We are seeking a Community Health Registered Nurse who is committed to client-centered care, and passionate about working collaboratively to address the social determinants of health, to reduce barriers and improve access to comprehensive health care for the residents or Thorncliffe Park, Flemingdon Park and North York.
