Salary Range: $40.24 – $57.68 per hour
St. Joseph’s Health Care is willing to consider applicants who are eligible for the Community Commitment Program for Nurses (CCPN). This program provides $25,000 in grant funding to eligible RPNs/RNs/ Nurse Practitioners in exchange for a two‑year full‑time commitment. An ideal candidate will have a strong patient focus, experience in diabetes education and management, and exceptional communication skills. You will see patients in a multi‑disciplinary team environment, in person and virtually. This role offers a unique opportunity to make a significant impact on the lives of patients living with diabetes by providing comprehensive education, support, and guidance. You will work closely to provide optimum, individualized care in both group and one‑on‑one appointments to support individuals living with diabetes. You will have the opportunity to work with a large team of nurses, dietitians, pharmacists, physicians, administrative staff and others at a center recognized as a regional leader in diabetes care, providing care to patients with type 1, type 2 and gestational diabetes.
