  • Salary $64,314 to $80,925 per annum plus benefits (salary is commensurate on primary care exp.)

Operating out of six locations, Unison offers core services that include primary health care, counselling, health promotion, Early Years programs, legal services, harm reduction programs, housing assistance, adult protective services, Pathways to Education™ and a Diabetes Education Program. Unison’s mission is working together to deliver accessible and high‑quality health and community services that are integrated, respond to needs, build on strengths and inspire change.

We are seeking a candidate for the position of Community Health Nurse to join the Primary Health Care team at our Jane‑Trethewey and Oakwood‑Vaughan sites. The ideal candidate for this position will have a strong background in clinical primary care, experience in health teaching, advocacy and health promotion.

Job‑Specific Responsibilities:
  • Provision of primary care and wellness assessments across the lifespan including immunization, chronic disease monitoring, triage and assessment over the phone, point of care testing, venepuncture, counselling and assessment, and care of minor episodic illnesses in collaboration with physicians, nurse practitioners, and allied health professionals
  • Health teaching and advocacy for individuals within a harm reduction framework
  • Working collaboratively within a multidisciplinary team
  • Participation in team meetings, committees, and centre‑wide initiatives

Licensed as a Registered Nurse with the College of Nurses of Ontario with no restrictions to practice

  • Baccalaureate degree or diploma with equivalent experience
  • Strong clinical assessment skills
  • Strong documentation and communication skills
  • Ability to speak a second language is an asset
  • Sensitivity to the impact of social health determinants on a diverse, urban, low‑income community
  • Sensitivity to, and knowledge of the needs of a multicultural, multiracial, community
  • Experience serving Unison’s priority populations including people with mental health and substance use issues, seniors with complex needs, LGBTQ2IA+ persons, including persons needing trans care, and youth with sexual health needs facing barriers to access
  • Commitment to pro‑choice philosophy
  • Current RNAO membership with malpractice coverage is an asset.

Successful candidate must be available to work one evening shift per week
