Job summary

Alberta Health Services in Alberta (South Calgary Health Centre area) seeks a Registered Nurse, Case Manager/Interventionist to coordinate and deliver nursing care in the community. Registered Nurse - Case Manager/Interventionist

Are you a Registered Nurse looking to work in a dynamic, interdisciplinary and community setting? The Registered Nurse, Case Manager/Interventionist position with the Integrated Home Care (IHC) program provides an exciting opportunity for you to work autonomously to your full scope of practice performing case management functions and providing direct clinical care.As a Registered Nurse, Case Manager/Interventionist, you will be responsible for coordinating and providing quality care to clients in the community. You will conduct nursing assessments, guide care planning, and provide case coordination and direct care in partnership with short term and long term clients and their families in their own residence or an alternate care setting. You will monitor services and ensure they are provided and modified as needed, based on client choice, effectiveness and authorization. It will be crucial to demonstrate proficiency in the following areas: personal and professional accountability, systems thinking, creative thinking, communicating and building relationships, and organizational and time management skills.The position requires flexibility, adaptability and travelling with your own vehicle. Manual handling (lifting, lowering, carrying, pushing and pulling) is a regular part of the job. For example: lifting items (approximately 30 pounds) in and out of a vehicle and carrying this weight up and down stairs throughout the day is common with this job. Learning resources and training are provided to safely perform work.

Description:

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values.PLEASE NOTE: Provisional Permit holders will be paid $40.76 to $55.79 per hour. Upon obtaining a full practice permit, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii), as applicable.

Required Qualifications:

Recent experience working in a Home Care Setting.Minimum of 3 years of recent nursing experience.A valid Driver’s License and a vehicle in good repair.Ability to lift 30 lbs.

Preferred Qualifications:

Experience working with acute clients and/or long term clients with a focus on chronic disease management.Experience providing case management, client teaching and professional nursing interventions including but not limited to Home Parenteral Therapy (HPT), wound care and ostomy care.Experience supporting vulnerable clients, those requiring palliative, mental health and/or addictions support.
