Job Summary:

Baycrest has been appointed as the health service provider accountable for leading the implementation of the Toronto Ontario Health regions’ Behaviour Support Action Plan. A key componentoftheplanisthedeploymentofa BehaviourSupportOutreachTeam(‘BSOT’),consisting of interdisciplinary community outreach clinicians, nurses and personal support workers who have behavioural support expertise. The nurse will travel to various community locations and long-term care (LTC) homes in the Toronto Ontario Health region and work with the inter-professional team to provide collaborative and evidence-based care.*This position will be predominantly working in the east end of the Toronto Ontario Health region.

Responsibilities include but are not limited to:
  • Providessoundclinicalknowledgeandskills,actsasclinicalexpertandresourceandprovides comprehensive, quality client/family centred health care services from admission, through transfers and discharge
  • Plansfor,deliversandevaluatesnon-pharmacologicalbehaviouralinterventionsforclientswith dementia and/or mental health conditions
  • Providesclient-centredcarewithininterdisciplinarycareteamtomeetthepersonalcare, activation, recreation and spiritual needs of a clients
  • AssistsstaffintheTorontoOntarioHealthLTCfacilitiestounderstandanddevelopbehaviour approaches to providing care to residents requiring behavioural support and their families.
  • CollaborativelydevelopsandplansassignmentswithPSWs,communityagenciesand LTC Teams

Qualifications include but are not limited to:

  • Minimum 1-year related experience in geriatrics, community based seniors care and behavioural management; three (3) years preferred
  • ExperienceworkingintheLTCandcommunitysectors preferred
  • TraininginGentlePersuasiveApproach,DementiAbilityand/orPIECES preferred
  • Psychogeriatricexperience,i.e.mentalhealthassessmentandinterventionandworkingwith responsive behaviours including various types of dementia, delirium, mental health issues preferred
