Flemingdon Health Centre (FHC) in Toronto is hiring a Registered Nurse to deliver client-centred primary care within an interdisciplinary team. The Registered Nurse will be an integral member of the interdisciplinary team, providing comprehensive, client-centred nursing services. Working within their full scope of practice and in accordance with the standards and requirements of the College of Nurses of Ontario (CNO), the RegisteredNursewill providenursingassessment,healthassessmentandmonitoring,careplanning, health education, clinical support, care coordination, health promotion, referrals, and navigation of communityandhealthresources.TheRegisteredNursewill helpreducebarriers tocareandaddress the social determinants of health while supporting clients in achieving improved health and well-being.

employedbyFHC, the incumbent willworkas part ofthe Unifycareteamand followUnify’s day-to-day site operations, schedules, and clinical workflows. Day-to-day direction will be provided onsite at Unify, while formal reporting and accountability will remain with FHC.
