Job summary

Flemingdon Health Centre (FHC) in Toronto is hiring a Registered Nurse to deliver client-centred primary care within an interdisciplinary team. The RN will perform assessments, planning, health promotion, and navigation of community resources, with emphasis on reducing barriers to care and addressing social determinants of health.

The role requires working within CNO standards, obtaining hours that may include evenings and weekends, and collaboration with Unify care teams under FHC leadership.

Qualifications

  • Registered Nurse with full scope of practice per CNO standards.
  • Bachelor of Science in Nursing (BScN) is required.
  • Minimum 1 year in primary care, with triage, chronic disease management and contraception counseling.

Responsibilities

  • Provide comprehensive primary care within RN scope.
  • Coordinate care with Health Centre and external providers.
  • Deliver telephone triage, advice, and follow-up.
  • Provide health education and promotion.
  • Support home visits for home-bound clients.

Background:Flemingdon Health Centre (FHC) is a not-for-profit Community Health Centre serving FlemingdonPark,FairviewandThorncliffePark.GuidedbyavisionofStrongHealthyCommunities, FHC provides health and community services grounded in health equity, inclusion, community engagement, accountability, excellence and collaboration.

FHCishiringaRegisteredNurse(RN) tobedeployedandworkatUnify.Thisispartofanexciting initiative where FHC is acting as a lead organization to support the delivery of primary care and integrated social services.

The Registered Nurse will be an integral member of the interdisciplinary team, providing comprehensive, client-centred nursing services. Working within their full scope of practice and in accordance with the standards and requirements of the College of Nurses of Ontario (CNO), the RegisteredNursewill providenursingassessment,healthassessmentandmonitoring,careplanning, health education, clinical support, care coordination, health promotion, referrals, and navigation of communityandhealthresources.TheRegisteredNursewill helpreducebarriers tocareandaddress the social determinants of health while supporting clients in achieving improved health and well-being.

DeploymentRole–EmploymentModel:WhiletheRegisteredNursewillbehiredandremain

employedbyFHC, the incumbent willworkas part ofthe Unifycareteamand followUnify’s day-to-day site operations, schedules, and clinical workflows. Day-to-day direction will be provided onsite at Unify, while formal reporting and accountability will remain with FHC.

ResponsibilitiesInclude:

  • Providetelephonetriage,advice,and follow-up onresultsandpreventionneeds.
  • Deliverclienteducationandhealthpromotionduringassessmentsandgroups.
  • Providehome visitsand supportforhome-boundclients,asneeded.

Health PromotionandEducation:

  • Workwithinternalteamstodelivertargetedhealtheducationandsupportcommunity development.
  • RepresenttheHealthCentreincommunityprojectsandpartner activities,as needed.
  • Supportoutreachprogramdevelopmentbasedoncommunity needs.
  • Contributetopoliciesandstrategiesthatadvancepublichealthandaddressdeterminantsof health.
  • Applyevidence-basedpracticealignedwithevolvingcommunity needs.

OrganizationalResponsibilities:

  • Participateinqualityimprovement,audits,accreditation,committees,andteam meetings.
  • Supportannual planningandevaluationofprogramgoals.
  • Identifyprioritypopulationsandhelpimplementeffectiveservices.
  • Maintainknowledgeofcommunityservicesandmakeappropriatereferrals.
  • Teach/superviselearnersandsupportvolunteerplacements,as needed.
  • Participateinprofessionaldevelopmentand staffin-servicetraining.

  • Strongassessment,healthpromotion/education,counseling,andcommunitydevelopment skills
  • Abilitytoworkeffectivelyonaninterdisciplinaryteam
  • Abilitytoworkflexible hours,includingeveningsandweekends.
  • Additionallanguages,preferablySpanishorArabic,andEMR/PracticeSolutionsexperience are assets.
  • Working Conditions&PhysicalRequirements:
