Overview

The Carefirst Family Health Team (FHT) is seeking Full-time permanent / Part-time permanent / Casual Registered Nurse / Registered Practical Nurse to join our innovative, interdisciplinary team dedicated to delivering high-quality, community-based care.

About Us

Carefirst FHT is committed to delivering exceptional, comprehensive primary care through a collaborative, team-based model that includes family physicians, nurse practitioners, nurses, social workers, dieticians, physiotherapists, and medical receptionists. We have a robust Chronic Disease Management and Prevention program, including a Diabetes Education Program, and are actively involved in many of the Ontario Health Team’s primary care initiatives.

Our strong community partnerships ensure coordinated, high-quality, patient-centered care across the continuum. We work closely with the Carefirst Seniors & Community Services Association to provide truly integrated services.

On-site & Integrated Programs/Services include:

  • Ontario Health atHome (formerly Home and Community Care Support Services)
  • MINT Memory Clinic
  • Ontario Structured Psychotherapy (OSP) Program
  • Specialist clinics: psychiatry, cardiology, endocrinology, nephrology, optometry, geriatric medicine, gynecology, chiropody, and audiology
Clinic Locations

We currently operate two well-established clinics in Scarborough and Richmond Hill. Beginning in Fall 2025, we will be expanding with five new locations to support continued growth and community needs.

Current clinic locations:

  • 300 Silver Star Blvd, Scarborough (Midland / McNicoll)
  • 420 Hwy 7 East, Richmond Hill (West Beaver Creek / Hwy 7)

New clinic locations:

  • Within Richmond Hill / Thornhill
  • 360 Highway 7 East, Richmond Hill (Highway 7 / Chalmers Rd)
  • 105 Gibson Centre, Markham (Steeles Ave East/Warden Ave)
  • Unionville Commons, Markham (Unionville Main Street/Highway 7 East)

Employees will be assigned to a location that aligns with their preferences and residence, with occasional rotation to support clinic operations.

We are currently seeking a Registered Nurse / Registered Practical Nurse to join our Family Health Team.

Terms: Full Time Permanent / Part Time Permanent / Casual

Responsibilities
  • Implement and coordinate a management system and interventions in support of the Chronic Disease Management and Prevention programs through outreach, health education, individual counselling, and virtual care
  • Provide clinical support while working closely with others in a multidisciplinary team
  • Engage in and facilitate activities under health promotion/teaching/prevention such as group education sessions and community outreach
  • Liaise with care providers in the health care system and facilitate referrals to other services and resources within the Family Health Team, secondary and/or tertiary health care centres
  • Ensuring practice is consistent with professional standards and the ethical framework as outlined by the College of Nurses of Ontario and organizational policy, practice and values.
  • Demonstrating commitment to professional lifelong learning by participating in continuing education activities to meet the needs of the patient population served and the standards of the profession.
  • Performing reflective practice, analyzing and evaluating nursing knowledge and modifying practice accordingly to improve the quality of patient care.
  • Performing cross-functional and other responsibilities, as assigned and/or requested
  • Promote positive public relations and working relations in a family-centered care environment
  • Provide administrative and clinic management support including but not limited to providing clinical coaching/support to the team and ordering and maintaining clinical/lab supplies, medications, vaccines, crash cart, PPE inventory, etc.
  • Maintain and manage patient files, charts, and records while prioritizing patient confidentiality
Requirements
  • Bachelor of Science in Nursing or RPN diploma from an accredited institution
  • Registered and in good standing with the College of Nurses of Ontario
  • Valid CPR HCP certification
  • Minimum 2 years of clinical experience in a community or primary healthcare setting
  • Demonstrated experience in the area of chronic disease management/ health teaching
  • Exceptional telephone etiquette and customer service skills
  • A team player with a high degree of accuracy and attention to detail
  • Strong interpersonal skills, with the ability to accept work from various sources (physicians, nurses, team leads)
  • Well organized and capable to multitask in a fast-paced environment
  • Proficient computer skills in Microsoft Office, and EMR (Telus Practice Solutions) is an asset
  • Available to work flexible hours including some evenings and Saturdays

Available to work at both the Scarborough and Richmond Hill clinic locations

  • Fluent in English and a second language that reflects the community served, such as Cantonese and Mandarin, is an asset
  • A valid vulnerable sector police record check completed within one year
