Job description

Are you an inquisitive, energetic and compassionate Registered Nurse who has a passion for caring for a diverse population? At Maple Family Health Team, you will be working closely with Family Physicians and learning from and collaborating with supportive fellow Registered Nurses and other Interdisciplinary Care Professionals.

We require an experienced Registered Nurse to join our multidisciplinary team, working 3 days (24 hours) per week. If you are interested in expanding into primary care, or if you are already experienced in the primary care nursing role, this is an excellent opportunity to join Maple Family Health Team.

Maple Family Health Team is a primary care organization that brings together health practitioners to provide and coordinate the best possible care for patients. The clinic, which provides care to the patients of five Maple physicians, is looking for a Registered Nurse with experience or interest in primary care nursing.

Position Description

The Registered Nurse (RN) provides primary nursing care to patients of Maple Family Health Team. Working closely with physicians, administrative staff and Interdisciplinary Healthcare Providers, RNs examine, interview, educate, dispense advice to and treat patients in keeping with Maple Family Health Team policies, procedures and values.

Representative Duties and Responsibilities
  • Participate in caring for patients of all ages, including prenatal, infant, child and eldercare.
  • Collaborate with team members to plan, implement, coordinate and evaluate patient care.
  • Support Maple FHT programs and services.
  • Use evidence‑based practice to develop the most appropriate plan of care and interventions.
  • Participate in team meetings and education opportunities.
Required Skills and Knowledge
  • Knowledge of community resources available to patients in the City of Kingston.
  • Able to assess and prioritize multiple requests by patients, families and team members.
  • Training and experience in smoking cessation counselling is an asset.
  • Experience providing care and education to people diagnosed with or at risk of the following: Diabetes, Lung Disease, Dementia, Hypertension and Osteoporosis.
  • Experience providing prenatal and postpartum care is an asset.
  • Experience delivering periodic health exams to all demographics.
  • Able to maintain confidentiality of sensitive patient information.
  • Experience using Electronic Medical Records.
  • Recent experience working in primary or community care (preferred).
Qualifications
  • Current unrestricted registration with the CNO as a Registered Nurse.
Information about the position
  • This is a permanent, part‑time (0.6 FTE) position.
  • HOOPP benefits begin immediately. Dental and Extended Health benefits begin after 4 months of employment.
  • The incumbent must be compliant with Maple’s COVID Vaccine Policy which requires full vaccination.
  • This position will begin as early as December 1, 2025.
