Job summary

The Central Community Health Centre in St Thomas is seeking a Registered Nurse to provide nursing assessment, health education, disease prevention, and chronic disease management within the RN scope of practice. You will work with an interdisciplinary team across site locations and mobile units.

The role includes immunizations, wound care, ECGs, documentation in the EMR, and coordination of referrals. Full-time, 37.5 hours/week, with competitive hourly pay and HOOPP benefits.

Qualifications

• Registered Nurse in good standing with the College of Nurses of Ontario (CNO).
• Experience in primary care or community health settings preferred.
• Ability to work within RN scope and collaborate with interdisciplinary team.

Responsibilities

• Provide nursing interventions within RN scope and follow policies, protocols, and legislation.
• Perform nursing assessments, triage, health histories, health education, counselling, and other clinical activities.
• Perform clinical procedures such as immunizations, injections, specimen collection, wound assessment and care, ECGs, and pulmonary function testing.
• Recognize changes in client health status and initiate appropriate nursing interventions, consultation, referral, or escalation of care.
• Monitor and support clients with acute and chronic health conditions, including preventative care and chronic disease management.
• Collaborate with clients, families, and the interdisciplinary team to develop, implement, and evaluate individualized plans of care.
• Coordinate referrals and follow-up with internal programs, specialists, community agencies, hospitals, and other health and social services.
• Advocate for clients experiencing barriers to accessing health care or social supports and build relationships with community partners.
• Maintain clinical areas, equipment, medications, vaccines, and supplies in accordance with established processes.
• Maintain accurate and timely documentation in the electronic medical record and appropriately action clinical results, correspondence, and referrals within scope.
• Participate in case conferences, chart reviews, quality assurance activities, team meetings, and program development.
• Support outreach and mobile services and perform other related duties as assigned.

Working within the RN scope of practice and in accordance with the standards of the College of Nurses of Ontario (CNO), the Registered Nurse provides nursing assessment, treatment, health education, disease prevention, chronic disease management, care coordination, and health promotion services. The RN also contributes to the development, implementation, and evaluation of CCHC programs and services and may provide care at CCHC, aboard the Mobile Health Unit, at community outreach locations, and through other community-based initiatives.

Employment Conditions

• Wage: $40.00–$45.00 per hour, based on experience and qualifications
• Full-time position, 37.5 hours per week • Eligible for HOOPP pension and benefits, in accordance with CCHC policy • Conditional upon satisfactory references and required screening
• Applicants must be legally entitled to work in Canada
• The normal work week consists of 37.5 hours, with flexibility to include occasional evening and/or weekend hours
• Satisfactory Police Criminal Record Check or Vulnerable Sector Check
• Proof of immunization for COVID-19 and measles is required, subject to applicable accommodation under the Ontario Human Rights Code
