Job summary

A healthcare services provider in Ontario is seeking a responsible professional for conducting in-home visits to monitor quality services by Home Care Aides. The role requires strong interpersonal and communication skills, as well as a client-focused approach. Experienced candidates in senior services with a minimum of 2 years are preferred. The schedule is Monday through Friday, 8am to 5pm. Benefits include medical, dental, 401K matching, and more.

Qualifications

  • Minimum of 2 years’ experience working in the field of senior services or related social service.
  • Must have reliable transportation, valid driver’s license, and insurance for travel.

Responsibilities

  • Monitors client’s condition and performance of Home Care Aides.
  • Conducts home visits to new clients to review the Welcome Packet.
  • Prepares and completes evaluation reports and case notes.
  • Maintains schedules for timely completion of home visits.
  • Follows all Medicare, Medicaid, and HIPAA regulations.

Position Summary

This position will be responsible for conducting in-home visits at clients' homes to monitor and report on the quality of services being provided by Home Care Aides, and to determine whether existing care plans are adequate to meet client needs.

Schedule

Monday Through Friday - 8am - 5pm (Home Visits and Office work)

Responsibilities
  • Monitors client’s condition and performance of duties by Home Care Aides
  • Maintains all company and Department on Aging rules, regulations, and standards
  • Conducts home visits to new clients to welcome and review the Welcome Packet
  • Observes and evaluates Home Care Aides in the client's home as directed by Supervisor, to assure client Care Plans are being followed
  • Prepares and completes accurate evaluation reports and case notes as needed
  • Maintains schedules to assure timely completion of all assigned home visits
  • Attends weekly branch meetings
  • Maintains a high degree of confidentiality at all times due to access to sensitive information
  • Maintains regular, predictable, consistent attendance and is flexible to meet the needs of the departmentFollows all Medicare, Medicaid, and HIPAA regulations and requirements
  • Abides by all regulations, policies, procedures and standards
  • Performs other duties as assigned
  • Maintain positive internal and external customer service relationships
  • Maintains open lines of communication
  • Plans and organizes work effectively and ensures its completion
  • Meets all productivity requirements
  • Demonstrates team behavior and promotes a team-oriented environment

Qualifications

  • Must demonstrate strong interpersonal and communication skills
  • Must be client focused and customer service driven
  • Computer experience – Microsoft Word and Excel
  • Must have reliable transportation, valid driver’s license and insurance for travel within the branch service areas
