Job summary

Hamilton Urban Core Community Health Centre is seeking a Nurse Practitioner for on-site, temporary part-time work (0.5 FTE) within a dynamic Primary Health Care team focused on homelessness, mental health, and complex needs.

The NP will perform comprehensive assessments, diagnose, treat, prescribe, and educate clients while collaborating with an interdisciplinary team and community partners to improve access and outcomes.

Qualifications

  • BScN with Nurse Practitioner certification and registration with the Ontario College of Nurses (Extended Class).
  • Broad-based nursing experience with strong clinical, assessment, and problem-solving skills.
  • Experience in primary health care, community health settings.
  • Experience providing trauma-informed, harm reduction, and equity-based care.
  • Sensitivity to issues affecting clients accessing HUCCHC, including mental health, substance use, homelessness.
  • Experience working with immigrants, refugees, seniors, youth, and low-income populations.
  • Knowledge of government-funded services and supports for individuals with complex needs.
  • Familiarity with the Community Health Centre sector is an asset.
  • Strong interpersonal, communication, and collaboration skills.

Responsibilities

  • Conduct assessments, diagnoses, treatment, health education, and counseling per CNO Extended Class standards.

Position Description

The Nurse Practitioner is a key member of the Primary Health Care team, providing comprehensive, client-centered, trauma-informed, and culturally safe primary health care to individuals experiencing homelessness, mental health challenges, substance use, and other complex health and social needs.

Working collaboratively within an interdisciplinary team, the Nurse Practitioner provides comprehensive health assessments, diagnosis, treatment, prescribing, health promotion, disease prevention, and ongoing management of acute and chronic health conditions. The role supports individuals across diverse community-based settings, including residential treatment, mobile and outreach services, drop-in programs, street health, and other integrated and co-located services.

The Nurse Practitioner promotes equitable access to care by recognizing and addressing the social determinants of health, reducing barriers to health services, and adapting care to the individual needs, strengths, and circumstances of each client. Through a harm-reduction, recovery-oriented, and culturally responsive approach, the NP builds therapeutic relationships, supports continuity of care, and contributes to innovative models of accessible, community-based primary health care.

The Nurse Practitioner also contributes to interdisciplinary care planning, health education, prevention, and system navigation, working with clients and community partners to strengthen health outcomes, continuity of care, and connection to appropriate services and supports.

Key Responsibilities:
Clinical Care & Client Support
  • Working withing the full scope of a Nurse Practitioner, conduct assessments, diagnoses, treatment, health education, and counseling in alignment with the College of Nurses of Ontario (CNO) standards for Nurse Practitioners (Extended Class).
  • Provide ongoing monitoring, chronic disease management, and follow-up care for clients. Respond to episodic illnesses, complex cases, and crisis situations, ensuring timely interventions.
  • Maintain accurate and comprehensive documentation in electronic medical records (EMRs), participate in chart reviews, and engage in case conferencing.
Interdisciplinary Collaboration & Coordination
  • Work closely with interdisciplinary team members and external service providers to ensure coordinated, holistic care.
  • Develop, implement, and evaluate treatment, education, counseling, and health promotion programs for individuals, families, and the broader community.
  • Collaborate on screening programs, workshops, and health initiatives to address community needs.
  • Support outreach efforts to enhance access to primary health care, including mobile services and off-site visits.
  • Participate in community engagement and partnership tables as directed.
  • Provide supervision and mentorship to students, graduates, and learners, aligning with HUCCHC’s commitment to education and professional development.
  • Identify gaps in protocols and procedures, contributing to quality improvement initiatives.
  • Participate in committees, team, staff meetings, and program evaluations to enhance service delivery.
  • Maintain professional competency through continuing education, research, and peer collaboration.
  • Support systemic improvements to address barriers to care, particularly for marginalized and underserved populations
Other
  • Maintain and develop professional competence through appropriate continuing education (i.e. peer interaction, literature review, conferences, courses, etc.).
  • Work within the anti-racism, anti-oppression framework at Hamilton Urban Core.
  • Participate in Centre-wide activities such as the annual general meeting, client centered activities, annual events and all other special events and activities related to Hamilton Urban Core
  • Other duties as assigned.
Performance Targets
  • Serve approximately 10-12 clients per day based on a 7.5-hour workday (1.0 FTE).
  • Maintain 30-minute appointment durations, with flexibility for complex cases, mobile visits, and cultural interpretation needs.

8 hour shifts/ Monday - Friday 9:00 am-5:00 pm, 1 day per week 12:00 pm-8:00 pm/ Occasional Saturday 9:00 am-2:00 pm
