Primary Care Nurse Role
The Primary Care Nurse will use their clinical knowledge and skills to support both patients and clinicians through the various stages involved in the primary care process. Our goal is to build capacity in Primary Care, and therefore work with clinicians to take nursing tasks off their list, so they have more time to spend with more complex patients. With a new and developing model, the role of the Primary Care Nurse will change and evolve. The right individual will have the ability to help develop the scope of work and grow their experience. Continuous learning and feedback opportunities will be provided for all members of the Mentor Clinic team.
Expectations : Patient Care
- Pre-visit Staging of the Patient : Triaging and assisting / supporting Nurse Practitioners and Doctors with visits, researching all reports, lab work, letters and medications and entering the chart to support patient visits.
- Clinical Visit Supports : PAPs, Well Babies and Well Women checkups as well as vaccines and other assessments as requested by the Providers; taking patient vitals (height, weight, BMI, blood pressure) and conducting any pre-work as appropriate to patient symptoms, attributes or reasons for visit; providing charting supports to team members during visit as needed; providing patient education and documentation to support patient wellness, self-care, and information about tests / procedures.
- Follow Up Care : Follow up on tasks assigned by clinical team members (labs results, referrals, calls to health care providers etc.); review charts to identify patient screening requirements as per protocol (e.g. mammogram, PSA).
Mentorship, Education, and Process Improvement
- Develop positive working relationships / partnerships with other disciplines, agencies and health care professionals to promote coordinated service, quality care and access to appropriate resources for patients.
- Collaborate with members of an interdisciplinary health team such as family physicians, nurses, dietitians, occupational therapists, social workers and others to plan, implement, coordinate and evaluate patient care in consultation with patients and their families.
- Participate in meetings and Continuing Medical Education sessions as required.
- Assist with development and improvement of systems and processes used by all providers within the clinic.
Skills and Qualifications
- Must-have Bachelor of Nursing; clinical experience in a primary care environment; ability to maintain employee confidentiality during interactions with staff; solid computer skills with a working knowledge of the Telus Med-Access EMR and Microsoft Office; Nova Scotia RN license and Medical Liability Insurance; ability to work collaboratively with other health care providers.
- Nice to Have Family Practice Nursing Certificate.
