We are seeking dynamic and passionate Physiotherapists (PTs) to join Elizz by SE Health, an innovative and growing community care service that is part of SE Health. In this role, you will provide high-quality, client-centered rehabilitation services to individuals in the community while supporting a new hospital-to-home care initiative designed to improve the transition from hospital to home.

Working within a collaborative and interdisciplinary care model, you will partner with a diverse care team to support clients in achieving their health and recovery goals. You will help develop and implement holistic care plans that address clients’ physical, cognitive, social, emotional, and spiritual needs.

Requirements
  • A graduate of an accredited program in Physiotherapy and a member in good standing of the College of Physiotherapists of Alberta.
  • Recent experience in managing patients in acute/rehab or community-based rehabilitation programs is preferred.
  • Reliable, flexible, creative, self-directed professional, able to work effectively and collaboratively with rehab assistants as well as with a high-performance interdisciplinary rehabilitation team.
  • Sound knowledge of community resources is an asset.
  • Effective interpersonal, organizational and communication skills.
  • A positive work and attendance record is essential.
  • Ability to adapt to new computer applications.
  • This role follows a hybrid work approach where you are able to work remotely and with the clients in their acute/rehab or community-based rehabilitation setting.
  • A vehicle and valid Class 5 driver’s license and the ability to travel.
