Job description

Dear Colleagues in Care,

Are you a physician in the GTA with a passion for hospice palliative care?

Have you heard aboutJourney Home Hospice? Operated by Saint Elizabeth Foundation, the hospice has been serving people experiencing homelessness and structural vulnerability in the heart of downtown Toronto since 2018. There is an interdisciplinary team of physicians, nurses, personal support workers, complementary therapists, and volunteers with a trauma-informed, culturally safe, and harm-reducing approach to care.

Currently there are 13 beds, and we are hoping to expand our physician team! There are two physicians currently supporting the program, and we work on fee-for-service basis with two weeks rotations where the physician does onsite visits and covers on-call hours (typically there is limited after-hours calls with our staffing model).

This is a vibrant opportunity to be part of a unique and specialized model of care serving patients who are often overlooked in our healthcare system. Journey Home Hospice promotes a fun, engaging, and responsive environment with a focus on patient-centred care supported by highly trained staff. Professional development opportunities are also available if requested and the site participates in ongoing research and evaluation.

If you or anyone in your network is interested in joining this dynamic team, please feel free to reach out to MatthewDurham@sehc.com, the Executive Director.
