Job summary

Optimal Care is seeking a Physical Therapy Assistant (PTA) for home health in the Grand Rapids area. You will perform PT services in patients' homes under a physical therapist's plan of care, with a focus on pain relief, functional restoration, and safe mobility.

Role requires PTA program completion and licensure, plus strong assessment skills and EMR proficiency. Prior hospital or home health experience is a plus; travel and valid auto insurance are expected.

Qualifications

• Completed Physical Therapy Assistant program approved by APTA.
• PTA license recognized in the practicing state.
• Knowledge of growth and development across the lifespan.
• Ability to assess data reflecting client status and interpret information for age-specific needs.
• Computer skills for using electronic medical records effectively.

Responsibilities

• Provide direct physical therapy services under supervision of physical therapist.
• Treat patients to relieve pain, restore function, and maintain maximum performance.
• Assist patients in therapeutic exercises, gait, transfer, and functional training.
• Utilize therapeutic modalities (heat, cold, ultrasound, massage, etc.).
• Observe and report patient response to treatment to the supervising PT and organizational leadership.

PHYSICAL THERAPY ASSISTANT - PTA

LOCATION: Grand Rapids, MI & Surrounding Area

SCHEDULE: Full Time, 8:00 am – 5:00 pm, Monday through Friday W/ Weekend Rotation

Key Responsibilities

The Physical Therapy Assistant administers physical therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician or advanced practice practitioner’s orders and plan of care under the direction and supervision of the Home Health leadership team.

In this role you will be responsible for:

• Provides direct physical therapy services planned, delegated and supervised according to directions of physical therapist and in accordance with the physician’s plan of care.
• May treat patient to relieve pain, develop or restore function, and maintain maximum performance.
• Directs and aids patients in active and passive therapeutic exercises, muscle reeducation, gait, and functional training, transfer activities and prosthetic training.
• Utilizes physical agents, including but not limited to heat, cold, water, light, electricity, ultrasound and massage.
• Observes and reports to the physical therapist and the Organization personnel the patient's reaction to treatment and any changes in patient's condition.
• Instructs patients and families in care and use of DME.
• Maintains necessary records; clinical, conference, and progress notes, which will be incorporated into the patient's clinical record in prescribed time frames

Required Qualifications

• Successful completion of a physical therapy assistant program at a school of physical therapy approved by the American Physical Therapy Association
• Physical Therapy Assistant License recognized in the state in which practicing
• Demonstrates knowledge and skills necessary to provide care to and communicate with the adult and geriatric populations
• Demonstrates knowledge of the principles of growth and development over the life span
• Able to assess data reflecting the client's status and interpret the appropriate information needed to identify each client's requirements relative to their age-specific needs
• Demonstrates computer skills sufficient to use the electronic medical record effectively
• Reliable transportation and valid automobile insurance

Desired Qualifications

• 2 years experience working as a Physical Therapy Assistant - hospital and/or home care experience preferred

Pay Range

$30 - $34 USD
