Job description

The Joint Venture partnership of San Gorgonio Memorial Hospital Outpatient Physical Therapy with Loma Linda Health system offers the therapists the unique opportunity to serve the community fulfilling the Missions and Visions of both the organizations with a wholesome approach.

The Physical Therapist will be responsible for establishing and maintaining a comprehensive, individualized interdisciplinary plan of care under Physicians’ orders, in an outpatient setting, which includes performing evaluations, integrative treatment planning, setting patient‑specific functional goals, follow‑up assessments, and ongoing patient/ family education; coordinate patient’s Plan of Care with other Medical and Rehabilitation Staff; provide care and maintain EMR in accordance with Physical Therapy departmental policies and procedures, JCAHO standards, Federal and State regulations, Medicare, Medi‑Cal, and primary insurance standards; foster a constructive and supportive approach to department operations, understanding economic constraints and practices designed to conserve resources, promote professional and collegial relationships; participate in Process improvement and Quality improvement measures; provide appropriate supervision and support to Physical Therapy Assistants and Rehabilitation Technicians; act as a clinical educator for students and other clinical staff.

Required:

  • Graduation from an accredited program of physical therapy, or foreign trained Physical therapy with State Board Licensure.
  • Current California Board of Physical Therapy licensure.

  • Estimated pay is based on qualifications (including but not limited to experience, education, certifications, and licensure where applicable).
