Qualifications

  • Bachelor of Science in Pharmacy from a recognized institution.
  • Licensed Pharmacist in good standing in the province.
  • Injection and First Aid Certification.
  • Additional certifications (e.g., Certified Geriatric Pharmacist, Advanced Prescribing).
  • Experience with Kroll software is preferred.

Responsibilities

  • Demonstrate warmth, empathy, and compassion while delivering patient-centered care.
  • Provide clinical services focused on continuing care obligations (e.g., disease state management, medication reviews, full scope of practice, reporting, after-hours emergency service); occasionally required to be on-site.
  • Collaborate with colleagues to deliver outstanding health and wellness services to all patients.
  • Initiate care plans, follow up, and complete accurate documentation.
  • Identify actual and potential drug-related problems through comprehensive medication reviews.
  • Educate nursing staff on medication-related topics.
  • Participate in facility committees and conduct biannual medication reviews as part of the care team.
  • Ensure prescription safety by prioritizing technical functions assigned to Pharmacy Registered Technicians and Assistants as needed.
  • Provide quarterly reports to facility administration, review medication errors, and complete audits.
  • Understand all relevant corporate pharmacy service offerings and ensure each patient receives the program that best meets their healthcare needs.
  • Stay current with pharmacy practice, regulatory requirements, and industry developments; attend required training.
  • Adhere to all regulatory requirements and company policies, processes, and procedures.

  • Stay current with pharmacy practice, regulatory requirements, and industry developments; attend required training.
  • Adhere to all regulatory requirements and company policies, processes, and procedures (e.g., OHS, Loss Prevention, CQI).
What you bring to the table:
  • A Bachelor of Science in Pharmacy from a recognized institution.
  • A Licensed Pharmacist in good standing in the province of responsibility.
  • Injection and First Aid Certification.
  • Additional certifications (e.g., Certified Geriatric Pharmacist, Advanced Prescribing).
  • Experience with Kroll software is preferred.
