We currently have an exciting Relief Full Time opportunity for a Pharmacist to join our team at Lions Gate Hospital located in North Vancouver / Pemberton / Squamish (North Shore), BC. This relief role is available until July 2026 or until return of incumbent.

Detailed Overview

The Pharmacist provides distribution pharmacy services by performing duties such as interpreting medication orders, assessing prescriptions for therapeutic suitability and ensuring that medications are efficiently and accurately dispensed and/or compounded and labelled and that dispensing records are complete. Communicates with physicians, patients and/or other health care team members for prescription clarification and problem resolution. Provides instructions to others on proper use, storage and disposal, possible side effects of prescribed and self‑selected medications. Participates in the collection of data for medication use evaluation projects, implementation of dispensary based medication safety initiatives, quality control activities, contributes to internal publications and participates in clinical drug trials in accordance with established standards and procedures.

Additionally, the Pharmacist functions as a Clinical Pharmacist responsible for ensuring select patient drug related problems are resolved. Participates in nursing ward rounds and/or program team meetings and documents patient specific therapeutic plans.

Responsibilities
  1. Interpret medication orders/prescriptions and dispense medications using a computerized medication information system in accordance with legal, professional, and departmental policies, procedures and standards.
  2. Assess prescriptions for therapeutic suitability by referring to standardized references and interpreting laboratory data.
  3. Review patient profiles for any allergies, potential drug interactions, therapeutic duplications or contraindications and withhold medications until safety concerns are resolved.
  4. Communicate with physicians, patients and/or other health care team members for prescription clarification and document the outcome of the interaction.
  5. Provide instructions to others on proper use, storage and disposal, possible side effects of prescribed medications.
  6. Participate in obtaining medication history from select patients and verify allergy information and assist with medication reconciliation process.
  7. Ensure that medications are efficiently and accurately dispensed and/or compounded, labelled and that dispensing records are completed.
  8. Provide medication counselling regarding drug therapy to patients and/or their families, document patient interactions in a standardized format.
  9. Review patient screening reports or targeted drug lists and identify potential drug related problems based on available laboratory data.
  10. Provide basic pharmacokinetic interpretations on patients receiving targeted drugs.
  11. Participate in the collection of data for medication use evaluation projects and document the required information.
  12. Participate in dispensary based implementation of medication safety initiatives and quality control activities in accordance with established standards and procedures.
  13. Maintain records for narcotic and controlled drugs, special access program drugs and departmental statistics in accordance with legal, professional and departmental standards, policies and procedures.
  14. Participate in the orientation of pharmacy students, residents, technicians, assistants and new staff members by demonstrating dispensary related functions, procedures, techniques, equipment and by providing verbal guidance.
  15. Contribute to internal publications and participate in clinical drug trials by dispensing medications and completing associated documentation.
  16. Participate in staff meetings, committees and other programs as required; identify opportunities to improve processes and/or procedures to enable safe and efficient operations and refer to supervisor and/or others.
  17. Perform other related duties as assigned.
Qualifications

Education and Experience

Current registration as a Pharmacist with the College of Pharmacists of British Columbia.
