To help fulfill its mission, LOFT is hiring 4 Relief Personal Support Workers.

Note: This role requires overnight shifts from 11:15 PM to 9:15 AM, covering weekends.

Job Summary

The Personal Support Worker is responsible for rendering a range of services for the clients of the program consistent with its basic values and goals. The PSW will support the needs of vulnerable, at-risk, older adults and seniors living in the multi-sites under the program. This support relates to the physical, cognitive, mental health and social/recreational needs of the clients. The staff member will maintain the established routines of the program including all security, fire and general health and safety practices. The PSW will perform these and other delegated responsibilities to ensure the quality of personal care of the clients of the program.

O’Connor provides 24/7 ALC support to male identified clients recently discharged from CAMH and provides a safe, supportive environment where the individuals can reintegrate into the community and develop life skills. The site is comprised of a Program Coordinator and 6 PSWs working in partnership with CAMH.

What You Will Do
  • Support the physical and mental health requirements of clients.
  • Establish a working rapport with clients that supports their dignity, encourages personal empowerment and facilitates addressing their psychosocial, behaviour management and medical needs.
  • Support clients with reminders related to taking their medications at the scheduled time(s) and in accordance with their physicians' instructions.
  • Adhere to residence/program established medication system procedures, including ordering, recording and reminding clients about the administration of their medication as well as the self-medication program.
  • Monitor and support clients with medical problems and illnesses, including cleaning areas which require immediate attention, e.g. linens, floor coverings, etc., and record illnesses, the circumstances and arrange for medical/emergency intervention in liaison with the Senior PSW and Program Director.
  • Encourage and assist clients in activities of daily living: laundering and ironing of clothing, dressing/undressing, personal hygiene, ambulation, toileting, meal preparation and feeding, care for incontinent clients, shopping.
  • Promote a normalized lifestyle by supporting daily living chores such as bed making, dusting, sweeping and keeping living spaces clean and orderly.
  • Provide support with managing behaviours and problems arising from cognitive issues.
  • Provide social support in areas such as conversation, making phone calls, writing letters, facilitating hobbies and other recreational activities.
  • Escort clients to medical appointments and social activities in the community.
  • Maintain current and accurate records of care services and document behaviours for individual clients per procedures.
  • Participate in regular meetings to develop and follow goals of support for clients.
  1. Maintenance and security, fire and general health and safety
  • In emergency situations, contact appropriate emergency services and record incidents; coordinate with Program Director as per procedures.
  • Interact with clients during shifts; daily formal contact to ensure client comfort and safety.
  • Record missing clients and follow program procedures.
  • Ensure equipment and supplies for personal support are available; report damages to maintenance and the Program Director.
  1. Specific medical responsibilities (at applicable sites)
  • Monitor the medication program and liaise with physicians, pharmaceutical consultants, other PSWs and the Program Director regarding system problems.
  • Ensure proper storage and recording of medication from the pharmacy in consultation with the pharmacist and physicians.
  1. Social/ Recreational Activities
  • Attend program coordination meetings and participate in planning activities; maintain accurate records of program participants.
  • Keep staff informed of client absences for meals/medication due to outings.
  • Ensure equipment and supplies are available and report any damage/maintenance needs.
  • Participate in all planned program activities and drive the van for client outings as required.
  • Other responsibilities as delegated by the Services Coordinator or Program Manager.
  • Answer the telephone and convey messages; perform related clerical tasks; convey emergency calls from satellite sites to on-call staff.
  • Maintain basic daily operation of the residence in the absence of senior staff and assist with emergency evacuation as applicable.
  • Participate in client activities, staff meetings, training and the occupational health and safety committee.
  • PSWs work rotating shifts across multi-housing sites and support the clients living there.
  • Accept other delegated assignments related to program operation.
What You Bring

Must Haves

  • Post-secondary education in human services (diploma or certificate such as Gerontology Worker Diploma, Personal Support Worker Certificate, etc.).
  • Experience with vulnerable older adults and seniors with cognitive impairments and/or mental health issues;
  • Knowledge of medication and general health and psychiatric conditions;
  • First Aid and CPR training;
  • Ability to organize activities and work with individuals and groups in assessment, problem solving and crisis management;
  • Ability to interact with clients in a positive and supportive manner;
  • Demonstrated ability to work with multi-disciplinary professionals and other staff in a team environment;
  • Good oral and written English skills;
  • Creativity and flexibility;
  • Computer skills;
  • Must be available to work evenings and weekends.

Nice-to-Haves

  • Possession of a valid Ontario Driver's License preferred; experience in social/recreational coordination preferred.
  • Understanding of cultural competency and anti-oppression framework;
  • Reflective of a multi-barriered group; a strong asset.
