Job summary

The Personal Support Worker (PSW) role focuses on compassionate, client-centered care delivered in clients’ homes or community settings, under supervision of a licensed professional.

Qualifications

  • Personal Support Worker Certificate (or equivalent qualification).
  • Strong interpersonal, communication, and problem-solving skills.
  • Clear vulnerable sector police check.
  • Experience in home care, retirement homes, or long-term care (asset).
  • Ability to work independently and manage multiple tasks in a fast-paced environment (asset).
  • Bilingual English & French (asset).

Responsibilities

  • Personal Care & ADLs assistance and support with daily living tasks.
  • Health monitoring and reporting changes to the supervising nurse.
  • Medication reminders or assistance per care plan.
  • Environmental support including light housekeeping, laundry, and meal prep.
  • Social and emotional support to reduce isolation.
  • Documentation and effective communication with clients and team.
  • Adherence to PPE, infection control, and safety regulations.
  • Team collaboration with nurses and care professionals.
  • Specialized care such as basic ostomy and catheter care; end-of-life and respite care as appropriate.
  • OH&S compliance and other duties assigned by supervisor.

Overview

The Personal Support Worker (PSW) provides essential personal care andassistanceto clients in their homes or community settings, supporting them with activities of daily living (ADLs) and promoting independence, dignity, and quality of life. Working under the direction of a regulated health care professional, the PSW plays a vital role in delivering compassionate, client-centred care in alignment withCarefor’smission and values.

Responsibilities

Client-centred support andassistancewith activities of daily living (ADLs), personal care and other care needs assigned. Duties will be carried out under the formal delegation of a registered nursing professional,in accordance withorganizational policies, applicable legislation, and within the scope of training and competence. Responsibilities may include, but are not limited to the following:

  • Personal Care & ADLs: Assist clients with bathing, grooming, dressing, toileting, mobility, and safe transfers (including use of lifts and mobility aids).
  • Health Monitoring:Observeand report changes in client condition to the supervising nurse or care team.
  • Medication Assistance: Provide medication reminders orassistanceas per care plan.
  • Environmental Support: Perform light housekeeping, laundry, and meal preparation tomaintaina safe and clean environment.
  • Social & Emotional Support: Engage clients in conversation and activities to reduce isolation and promote well-being.
  • Documentation & Communication: Accurately document provided care and communicate effectively with clients, families, and team members.
  • Safety & Infection Control: Follow Personal Protective Equipment (PPE) protocols, infection prevention practices, and health and safety regulations.
  • Team Collaboration: Work collaboratively with nurses, supervisors, and other health care professionals to ensure continuity of care.
  • Specialized care: provide basic ostomy and catheter care;assistwithend of lifecare and respite care where appropriate
  • Acts in compliance with the Occupational Health and Safety legislation, regulations, andCareforpolicies and procedures
  • Other related duties/tasks as assigned by the Supervisor, Personal Support Services
Qualifications
Vehicle & Licensing Requirement

As a condition of employment, certain Part‑Time positions require employees to have reliable access to a motor vehicle during working hours and to maintain a valid driver’s license. Employees are responsible for ensuring that any vehicle used for work purposes is safe, in good working order, and properly insured in accordance with applicable laws and organizational requirements.

  • Personal Support Worker Certificate (or equivalent qualification -Internationally educated nurse (IEN)/ RPN student – 1st year/ RN student – 2nd year )
  • Valid driver’s license and full access to a vehicle
  • Strong interpersonal, communication, and problem-solving skills
  • Clear vulnerable sector police check
  • Experience in home care, retirement homes, or long-term care (asset)
  • Ability to work independently and manage multiple tasks in a fast-paced environment (asset)
  • Ability to service clients in any geographical area when the need arises (asset)
  • Demonstrate ability to be flexible in accepting a variety of assignments (asset)
  • Gentle Persuasive Approach (GPA) (asset)
  • Enhanced Palliative Care certificate (asset)
  • Current CPR and First Aid certification (asset)
  • Registered PSW with Health and Supportive Care Providers Oversight Authority (HSCPOA) (asset)
