Job summary

Foundation Health is seeking a Personal Support Worker to provide compassionate in-home and community care across our client base in Etobicoke/Greater Toronto area. The role includes personal care, companionship, light housekeeping, meal prep, and supporting the client’s daily living activities.

Flexible scheduling, comprehensive benefits, and training are part of the package. Applicants should hold a PSW certificate or equivalent; additional health-care qualifications or RN/RPN status are

Qualifications

• Valid Personal Support Worker certificate from a recognized program.
• RN/RPN or related health-care student status acceptable.
• Home Support Worker or similar certification is acceptable.
• Immunization and health clearances as required.

Responsibilities

• Assist clients with personal care (bathing, dressing).
• Provide companionship and social engagement.
• Assist with outings, exercise, and meal preparation.
• Light housekeeping and laundry tasks as needed.
• Support medication reminders and routine care plans.
• Contribute to client total care program under scope of practice.

Foundation Health Job Description

Personal Support Workers (PSWs) will provide care to individuals who need assistance as they deal with the effects of aging, injury, surgery, or illness.

Please note that this position is for agency staffing support and home care services.

The Role Will Include

• Assisting clients with personal care such as bathing and dressing
• Providing companionship
• Taking clients for outings or exercise
• Performing basic meal preparation
• Performing light housekeeping duties such as washing dishes, taking out the garbage, doing patient's laundry, vacuuming and dusting
• Providing respite while the primary caregiver is out of the home
• Reminding clients about their medication
• Taking clients to appointments or shopping
• Being a part of the client's total care program
• Acting upon delegated tasks as required within your scope of practice

Qualifications

• Successful candidates must have one of the following qualifications:
  • Valid Personal Support Worker Certificate from a Ministry of Training, Colleges, and Universities or the Ministry of Education approved program
  • RN/RPN, Paramedic, Occupational Therapy and Physiotherapy student with a minimum successful completion of year one of the program
  • Holds a valid certificate as a Home Support Worker (level II or III), Health Care Aide, or Personal Attendant
  • Holds a valid certificate as a Developmental Support Worker
  • Internationally licensed Nurse, Midwife or Physician
• Valid G2/G driver's license and access to a reliable car is an asset
• Physical ability to stand, bend, walk, crouch, kneel and reach, ability to lift 50lbs (23 kg) independently, ability to assist clients with standing, walking, and sitting; ability to reposition clients in bed
• Valid Standard First Aid and Level C CPR
• Updated immunization records (including completion of a two-step tuberculosis test within the last 6 months)
• Clear criminal background check, and Vulnerable sector check are required upon hire
• Ability to interact with others in a caring, tactful and courteous manner
• Proficient verbal and written English communication skills
• Ability to use computer and phone applications

Requirements

• Vulnerable Sector Check completed within the last six months
• Submission of proof of three COVID vaccinations
• TB test completed within 6 months to a year
• Immunization record
• Up-to-date CPR and First Aid certification
