Qualifications

As a member of an inter‑professional team reporting to the Care Manager, the role of the Personal Support Worker (PSW) is to ensure the provision of excellent care and support to the patients of hospice house, their families and caregivers, as outlined by hospice house and organizational policies and procedures. The PSW is responsible for providing routine care for all activities of daily living, support patients and families, and support volunteers as needed. The PSW assists in maintaining a welcoming home‑like environment contributing to daily operations of the program in accordance with hospice house policies and procedures. The PSW supports the efficient operation of the house through continuous quality improvement and fulfills their duties in a manner that is consistent with the mission, vision and values of the Providence Care Centre.

Skills & Qualifications Required
  • Diploma or certificate (in accordance with the Ontario Qualifications Framework) from an accredited PSW program or from a private career college or Adult and Continuing Education School Board Program.
  • Palliative Care Certificate – such as LEAP for PSW’s (Learning Essential Approaches in Palliative Care), or PACE for PSW’s (Palliative Care Education) or a willingness to complete within 6 months of hire.
  • Valid CPR/First Aid Certification.
  • At least 2 years demonstrated experience providing hospice palliative care in community, hospice, hospital or long‑term care setting.
  • Prior experience in pain or symptom assessment and management.
  • Prior experience working with an interdisciplinary team and supporting volunteers.
Position Responsibilities
  • Provide direct care to the patient according to the wishes of the patient and their care plan.
  • Bath the patient in accordance with the care plan and patient preference as requested.
  • Provide oral, nail and hair care as requested and in accordance with the care plan.
  • Apply topical creams and ointments according to pharmacy instructions and apply protective dressings.
  • Assist with or dress patient daily according to patient preferences.
  • Provide bedtime care consistent with the care plan and patient preference.
  • Assist patients to the bathroom or provide commode, bedpan or urinal in a timely manner.
  • Provide incontinence care, colostomy care or empty catheter drainage bags.
  • Assist patients requiring help to move from one area to another according to care plan.
