The Personal Support Worker (PSW) is part of an interdisciplinary team providing direct care, intervention, and support to older adults living in a long-term care home (LTCH). Clients include those demonstrating responsive behaviours associated with dementia, delirium, mental health, addictions, or other neurological conditions.

The PSW provides assistance with personal care and activities of daily living for program clients. Working collaboratively with the MRT Registered Nurse and Registered Practical Nurse, the PSW enhances clients' quality of life by participating in the assessment of responsive behaviours, providing direct care, identifying triggers, and supporting strategies for managing responsive behaviours in collaboration with LTCH staff. All work aligns with the Mission, Vision, and Values of Providence Care Centre.

Skills and Qualifications Required:

• Certification as a Personal Support Worker as per the Long-Term Care Homes Act and Regulations
• Minimum two years of experience in long-term care settings; relevant experience may also be considered
• Satisfactory criminal record and vulnerable-sector screening
• Effective teamwork and communication skills
• Bilingual in English and French preferred

Position Responsibilities:

• Provide person-centred care and assist with activities of daily living as directed by the MRT RN or RPN, in collaboration with LTCH team members
• Responsibilities may include personal care, monitoring fluid and nutrition intake, reporting responses to pain management, bowel and bladder care, exercise, positioning, ambulation, and ensuring client safety and comfort
• Collaborate with Behavioral Support Services, Outreach Teams, and other partners
• Develop therapeutic relationships with clients, families, and care providers using a person-centred approach
• Follow directions from the RN/RPN regarding care priorities and plans, offering suggestions for strategies supporting responsive behaviours
• Work in various LTCHs within the assigned region
