Job summary

A community service organization is seeking a Part-Time Personal Plan Facilitator in Sudbury. This role involves creating comprehensive Personal Plans by working closely with individuals and their support teams. French proficiency is required, and flexibility in working hours is essential.

Personal Plan Facilitator - Part-Time (20 hrs / week)


Rate of Pay: $29.48 - $33.30 / hr

The Personal Plan Facilitator works with the person using services their circle of support (including the Karis Disability Services team(s) that provides support) to learn about the person and use this information to facilitate the creation of a comprehensive Personal Plan. The plan will address the person's needs hopes and dreams with an eye toward building communities of belonging in accordance with the core Values of Karis Disability Services; Valuing People Fostering Belonging Serving Others and Respecting Gifts.

  • Driver's License

  • Communication Skills written & oral

  • Interpersonal Skills

  • French language proficiency is required (written and oral)

  • 5 years of experience in developmental services or other similar social services (e.g. childrens or seniors care).
