Job summary

Qualifications

• One to two years’ experience as a PSW.
• Current CPR and First Aid certification required.
• Demonstrated knowledge of fundamentals of Palliative Care.
• Current criminal background check including vulnerable sector screen.

Responsibilities

• Provide direct hands-on care to patients in their home.
• Document clinical data in the patient’s electronic medical record.
• Communicate and collaborate with the family/caregiver.

Strong written and verbal communication skills

Education
