Job summary

A community health organization is seeking a Registered/Licensed Practical Nurse in Northwestern Ontario to provide client-centered, evidence-based nursing care for medically fragile and complex patients. Responsibilities include managing care plans, documenting assessments, and educating clients and families. The ideal candidate must hold a nursing license and demonstrate strong communication and interpersonal skills. This position offers a full-time schedule with duties primarily based in clients' homes.

Qualifications

• Experience in community health is an asset.
• Clinical practice experience is beneficial.
• Additional degrees or certifications may be required for specialized positions.
