Job summary

Surrey Place, a not-for-profit serving Toronto region clients, seeks a Registered Nurse for Urgent Response Services. The role reports to the Manager, Complex Community Teams and focuses on assessment and rapid interventions for clients with autism and developmental disabilities for up to 12 weeks.

The nurse screens referrals, conducts health assessments, develops care plans, and collaborates with medical professionals to support families within a multidisciplinary team.

Qualifications

• RN registration in good standing with the College of Nurses of Ontario.
• BSc in Nursing or equivalent; willingness to pursue higher education.
• Minimum three years of community nursing experience preferred.
• Experience with developmental disabilities is an asset.
• Ability to work in a multi-disciplinary team and manage caseloads.
• Must provide a vulnerable sector police check; French is an asset.

Responsibilities

• Collaborate with URS team to triage and prioritize referrals.
• Perform comprehensive health assessments of infants to youth.
• Develop and evaluate plans of care within RN scope.
• Coordinate with physicians, NPs, and specialists for treatment.
• Support families and caregivers through education and resources.
• Document intake, intervention, and discharge in records.
• Provide nursing consultation to the client team on care impacts.
