Responsibilities

• Conduct OT assessments of physical and cognitive status and goals.
• Develop prognosis and evidence-based intervention plans.
• Educate patients and caregivers on self-management and health promotion.
• Participate in interprofessional care planning and care transfers.
• Document outcomes and adjust plans to achieve independence.
• Delegate tasks to rehabilitation assistants when appropriate.
• Contribute to research, quality and safety activities.
• Perform other related duties as assigned.
