Job Summary

The Registered Practical Nurse (RPN) plays a key role in supporting the healthcare team by conducting comprehensive assessments for PSW clients under the supervision of a Registered Nurse (RN). The primary responsibilities include visiting clients in their homes, hospitals, or other healthcare settings to evaluate their environment and overall well-being. The RPN will perform client assessments, develop personalized care plans, and collaborate with interdisciplinary team members to ensure the delivery of high-quality care and positive client outcome.

Responsibilities
  • Performs client assessment in the community
  • Provides accurate, detailed, and timely documentation
  • Conducts ongoing assessments
  • Create plan of care
  • Prepares, administers and appropriately documents actions taken specific to medications
  • Performs all skills, treatments, and procedures competently in accordance with the provincial scope of practice
  • Follows principles of infection control and Universal Precautions
Qualifications
  • Current certificate of competence and good standing
  • Current CPR/First Aid Certification.
  • Clear Police Record Check (Vulnerable Sector Screening) or willingness to obtain one.
  • Negative TB test or chest X-ray
  • Strong clinical skills
  • Self-directing with the ability to work with little direct supervision
  • Excellent customer service skills
  • Reliable internet access and a mobile phone.
  • Experience with Dementia/Alzheimer’s care is an asset.
