Position Summary

We are seeking a dedicated, compassionate and detail-oriented Licensed Practical Nurse (LPN) to join us part-time to conduct client health assessments, provide care, perform home safety checks, and carry out reassessments.

Key Responsibilities

• Conduct comprehensive health assessments for new clients
• Perform reassessments for existing clients as required
• Complete safety evaluations of client home environments
• Identify potential health risks and develop mitigation strategies
• Collaborate with caregivers, clients, and families to support safe care delivery
• Provide documentation, recommendations, and support for care plans
• Maintain accurate client health records in compliance with standards
• Support in developing personalized care recommendations
• Communicate findings clearly with the ComForCare team and stakeholders

Qualifications

• Active Licensed Practical Nurse (LPN) license in good standing with CLPNA
• Minimum 2 years of nursing experience in home care, long‑term care, or hospital settings
• Strong assessment and critical thinking skills
• Ability to work independently and communicate professionally
• Valid driver’s license and access to reliable transportation preferred
• Passion for senior care and person‑centered service
• Excellent communication skills and attention to detail
