Job summary

Qualifications

• Master's Degree in Physiotherapy and current registration with the College of Physiotherapists of Ontario.
• Minimum of three years of physiotherapy experience in a community setting.
• Strong commitment to community-based health care and health equity.

Responsibilities

• Assess client mobility and set goals with the interprofessional team.
• Provide education, screening, referrals and follow-up according to standards.
• Maintain complete, accurate client records in a timely manner.
• Support staff with consultation and health promotion activities.
• Participate in chart reviews, case conferences and data analysis.
• Contribute to developing primary care and health education programs.
