Job summary

Alberta Health Services in Calgary seeks an Occupational Therapist I to deliver comprehensive OT services within Home Care. You will work with an interprofessional team to optimize clients’ independence through targeted goals and personalized care plans.

Requirements include a Bachelor's in OT, ACOT registration, 3–5 years acute care and 1–2 years home care experience, BCLS-HCP, and a valid driver’s license. Eligible for temp F/T benefits; shift days with weekends off.

Qualifications

  • Completion of bachelor's degree in Occupational Therapy from an accredited university program.
  • Registered with the Alberta College of Occupational Therapists (ACOT).
  • 3 to 5 years Occupational Therapy experience with acute care focus.
  • 1 to 2 years recent community/Home Care based setting experience.
  • A vehicle in good repair.
  • Ability to lift 30 lbs.
  • Basic Cardiac Life Support – Health Care Provider (BCLS-HCP) within the last 12 months.
  • Home Care experience required.
  • Preferred: experience with mental health, addictions, and/or harm reduction.

Job description

This exciting community based Occupational Therapy Interventionist position reports directly to the designated Care Manager within the Calgary Zone Home Care Program, Adults Team. The Occupational Therapist works within the full scope of their practice as a member of an interdisciplinary health care team. Home Care uses a strength–based Client Centered Case Management practice framework. Using a person-centered approach, you will develop care plan(s) and provide direct care for client(s)/family(s) by identifying their needs, focusing on strengths, and setting achievable personal health and self-care goals. In collaboration with the client(s)/family(s) and interdisciplinary team you will ensure care and services are provided and modified on an ongoing basis, based on client’s response to interventions, incorporating stewardship into service provision.

Description:

As an Occupational Therapist (OT), you will provide evidence-informed services, with a focus on occupational performance designed to meet the needs of individuals, groups, or a community. You may act as a direct service provider, consultant, educator, or advocate and function as an integral health care team member. You will engage individuals, groups, and communities to enable meaningful and chosen occupations, including self-care, productivity and leisure. You will support clients to achieve optimum functional abilities and independence related to their cognitive, affective, spiritual, and physical strengths and limitations. As an OT, you will provide comprehensive services, including triage, screening, consultation, assessment, diagnosis, collaborative goal setting, treatment, prevention, self-management, education, case management, referral, transition support, and collaboration with relevant and chosen stakeholders and supports within the client¿s physical, institutional, cultural, and social environments. You will include clients and families as integral members of the health care team, encouraging their active participation in all aspects of care. You will collaborate with clients and families in determining, implementing, and evaluating rehabilitation. You will participate in a variety of interprofessional teams to build awareness, enrich support and engage in quality improvement by partnering with healthcare providers, patients, families, and other stakeholders to advance services for the clients. You will contribute to quality improvement and their professional community and participate in peer mentoring and supervising students. You may also provide clinical supervision to Therapy Assistants and Therapy Aides.

Required Qualifications:

Ability to lift 30 lbs.You will be required to operate a laptop computer and mobile phone, and for manual handling (lifting, lowering, carrying, pushing and pulling) as a regular part of the job.Basic Cardiac Life Support – Health Care Provider (BCLS-HCP) from Heart and Stroke Foundation within the last 12 months.Home Care experience required.
