Job summary

A healthcare service provider in Canada is seeking an Occupational Therapist dedicated to improving clients’ lives through integrated therapy services in their homes. This role requires a recognized qualification in Occupational Therapy and active registration with the Ontario College. You will develop individualized care plans, collaborate with families, and provide a hybrid care model. We are looking for an Occupational Therapist who can bring an integrated approach to therapy services in clients’ homes. Our Occupational Therapists work in a flexible and adaptable environment, as part of a team that values inclusivity, diversity, and evidence‑based practice.

What You Will Be Doing

• Change clients’ lives in the comfort of their home by assessing and building accurate therapy care plans based on each client’s real environment.
• Develop health‑promotion programs, prescribe equipment and supplies, and promote client safety.
• Exercise critical thinking to provide complex preventative or corrective actions for a variety of clients with diverse needs.
• Partner with clients’ family members, caregivers, and other health‑care professionals regarding treatment goals and service plans.
• Use CTG’s hybrid model of care—including face‑to‑face and virtual—to build a schedule that works for you and your clients.
• Operate as an independent and flexible OT while receiving dedicated support, mentorship and training from a stable multi‑disciplinary team.
