Job summary

Fraser Health is seeking an Occupational Therapist to work in a home-based unit, linking patients with care staff via technology from a central office. You will assess, plan, deliver, and monitor OT services, including community outreach and virtual care to patients in their homes.

You'll collaborate with mental health teams, educate patients and families, and contribute to program development while maintaining records and participating in ongoing professional learning.

Qualifications

• Bachelor's degree in Occupational Therapy from an accredited university.
• Two years' recent related experience working in a mental health setting, or an equivalent combination of education, training and experience.
• Current full registration with the College of Health and Care Professionals of BC.
• Eligible for membership in the Canadian Association of Occupational Therapists (CAOT) or the British Columbia Society of Occupational Therapists (BCSOT).
• Valid Class 5 Driver's license and access to personal vehicle for business-related purposes, as required.

Responsibilities

• Assess patient's occupational performance by reviewing records, interviewing the patient, family and team members.
• Plan individual and/or group occupational therapy treatment programs with outcomes and resources.
• Implement therapy programs using diverse approaches, possibly including home visits and virtual care.
• Monitor outcomes and use data to improve service delivery and program planning.
• Delegate components of treatment to assistants and supervise student learning.

This unique unit serves patients in their own home, using technology to connect them with the care staff that they would normally encounter in an inpatient setting. You would work out of a central office with your team, connecting virtually with patients. Detailed Overview

Reporting to the Manager, Clinical Operations for operational and administrative issues and coordination of program services including the development, delivery and evaluation of services outcomes; and to the Professional Practice Leader, Occupational Therapy for professional and practice issues. Duties include assessing, planning implementing and monitoring occupational therapy treatment programs, communicating with patients/families, other healthcare givers and the community, maintaining patient records and statistical data, delegating components of treatment to assistants and Mental Health Care Worker, facilitating student learning, preparing and delivering presentations, attending Mental Health Addictions departmental and hospital-wide in-services and educational programs and assisting with departmental maintenance and quality management. May provide care to patients through a combination of telephone, virtual video/audio conference and/or remote technology to patients in their homes, in the community including outreach, and virtually.

Responsibilities

1. Utilizing an evidence-based approach, assesses patient's occupational performance by methods such as reviewing records, interviewing the patient, family and team members, and gathering relevant information.
2. In collaboration with Mental Health and Addictions Team, plans individual and/or group occupational therapy treatment programs by establishing and prioritizing expected outcomes, discussing implementation options, and determining resources available.
3. Implements occupational therapy treatment programs utilizing diverse intervention approaches to promote, develop, maintain and/or restore occupational performance potential. May provide care to patients through a combination of telephone, virtual video/audio conference and/or remote technology to patients in their homes, in the community including outreach, and virtually when required in order to assess and/or engage with patients within their own environment and community.
4. Monitors the quality and outcome of occupational therapy treatment programs by determining the purpose, method and criteria of the evaluation, implementing the evaluation, and analyzing and interpreting results. Uses results for future planning and to improve service delivery. Utilizes available software/hardware applications for data collection and analysis.
5. Delegates' components of treatment to assistants/aides by instructing, observing and monitoring treatment provided to ensure quality patient care. Provides input for the performance evaluation of the assistants/aides.
6. Provides consultation, liaison and education to patients/family, other health team members and the community to ensure comprehensive service delivery, promote teamwork and avoid service duplication.
7. Maintains patient records, clinical outcome measures and statistical data in accordance with department, facility and professional standards.
8. Participates in interdisciplinary, departmental and professional development and education to maintain on-going professional competency.
9. Provides instruction, feedback and supervision of students to facilitate student learning, by modeling professional and ethical occupational therapy practice.
10. Prepares and delivers presentations on subjects related to Occupational Therapy to facility staff for inservice and orientations, and to the general public.
11. Assists with departmental maintenance, process structure, planning and quality management issues by assisting to develop and maintain occupational therapy programs, supportive documentation, resource material, and equipment and materials.
12. Performs other related duties as assigned.
