About the Opportunity

The transitional care model bridges hospital and home, supporting patients who need extra time and care to return to their community. The Nurse Practitioner works collaboratively to create evidence‑based care plans that help patients achieve their goal of going home safely. In this role you will also:

  • Work autonomously to diagnose, order, and interpret diagnostic tests
  • Prescribe pharmaceuticals and perform procedures within your scope of practice
  • Partner with physicians and care teams to support safe transitions from hospital to home
  • Collaborate closely with patients and families to build personalized care plans aligned with their goals
  • Contribute to quality improvement and innovation in transitional health care

  • A Master’s degree in Nursing with Nurse Practitioner Certification
  • Registration with the Nova Scotia College of Nursing for licensing (we can help!)
