Responsibilities
  • Provide direct patient care with a harm reduction approach, focusing on HIV and Hepatitis C treatment.
  • Conduct health assessments, diagnoses, and treatment planning.
  • Prescribe medications and monitor patient progress.
  • Educate and counsel patients on preventive care and chronic disease management.
  • Collaborate with physicians and healthcare teams to ensure comprehensive patient care.
  • Maintain compliance with CNO standards, medical directives, and privacy regulations.
  • Document patient interactions in electronic medical records (EMR).
  • Engage with community partners to enhance patient support services.
Qualifications

Registered Nurse Practitioner (NP-Adult or NP-Primary Health Care) with valid CNO registration.

Masters degree in Nursing (MN-NP) or Master of Science in Nursing (MScN).

Experience in primary care, harm reduction, HIV, Hepatitis C, or community health (preferred).

Strong clinical assessment, diagnostic, and prescribing skills.

Excellent communication and patient education abilities.

Ability to work independently and collaboratively in a multidisciplinary team.

Proficiency in electronic medical records (EMR) systems.

N95 Mask Fit Test Certification & up‑to‑date immunizations (MMRV, TB test, or chest X‑ray).
