Job description

We are seeking a compassionate and experienced Nurse Practitioner (NP) to join our client's team in a retirement living setting. The NP will provide comprehensive primary care to residents, collaborating with an interdisciplinary team to support health promotion, chronic disease management, and timely medical intervention. This role is pivotal in enhancing residents' quality of life and reducing unnecessary hospital visits.

Key Responsibilities:

Provide advanced nursing care, including assessment, diagnosis, and treatment planning.

Manage chronic conditions such as diabetes, hypertension, COPD, and dementia.

Perform routine health assessments and wellness checks.

Prescribe medications and treatments in accordance with NP scope of practice in Ontario.

Collaborate with physicians, nurses, PSWs, pharmacists, and other healthcare professionals.

Lead care conferences and communicate with families as needed.

Respond to urgent medical concerns and coordinate acute care when necessary.

Promote resident independence and preventative care strategies.

Maintain accurate and timely medical documentation in compliance with regulatory standards.

Support staff education and development in areas of clinical best practices.

Qualifications:

Registered Nurse in the Extended Class (NP) with the College of Nurses of Ontario (CNO).

Experience in geriatrics, primary care, or long-term/retirement care preferred.

Current CPR certification and clear vulnerable sector check.

Strong communication, clinical judgment, and leadership skills.

Familiarity with electronic medical records (EMRs) and Ontario healthcare regulations.
