Rexdale Community Health Centre (RCHC) seeks a Full-Time Hindi-Speaking Nurse Practitioner to join the Interprofessional Primary Care Team (IPCT). The role focuses on culturally responsive, comprehensive primary healthcare for diverse communities in North Etobicoke.
Responsibilities include clinical assessments, health promotion, disease prevention and management of chronic conditions, with travel to partner clinics as needed.
The Rexdale Community Health Centre (RCHC) is a multi-service centre providing primary health care, health promotion and community support services, including settlement and community development activities for families, children, youth, adults and seniors of diverse backgrounds. RCHC is recruiting for a Full-Time Hindi-Speaking Nurse Practitioner to support the work of the Interprofessional Primary Care Team (IPCT).
The IPCT is a collaborative initiative dedicated to providing culturally responsive, comprehensive, and holistic primary healthcare services to marginalized and underserved communities. Our team comprises diverse healthcare professionals committed to addressing the social determinants of health and achieving equitable health outcomes for all.
As a Primary Care Nurse Practitioner, you will play a vital role in delivering high-quality, patient-centered primary healthcare services to individuals and families from diverse communities. Working within our Interprofessional Primary Care Team (IPCT), you will provide clinical assessments, diagnosis, treatment, health promotion, disease prevention, and ongoing primary care services, with a focus on reducing barriers to care and improving health outcomes for underserved populations.
This position requires fluency in Hindi, a commitment to culturally responsive care, collaboration with interdisciplinary team members, and a passion for serving diverse communities. The Nurse Practitioner will work across partner clinics within the North Etobicoke IPCT Collaborative, providing accessible, coordinated, and comprehensive primary care services in a variety of community-based settings.
Fluency in Hindi and English (both verbal and written) is required
$108.091 to $130.231 annually (depending on experience) plus HOOPP and benefits
