Nurse Practitioner – Hepatitis C Program (Full-Time, Permanent)

Location: Scarborough, ON

Hours: 35 hours per week

Compensation: $51.29–$72.65/hour

About the Role

We are seeking a Nurse Practitioner with a passion for community health and harm reduction to join a multidisciplinary team supporting clients affected by Hepatitis C. This role focuses on education, diagnosis, treatment, and ongoing support—improving outcomes and reducing transmission through compassionate, evidence‑based care.

Key Responsibilities
  • Deliver client‑centered, trauma‑informed care within full NP scope of practice.
  • Provide diagnosis, treatment, and ongoing follow‑up for clients living with or at risk of HCV.
  • Support patients in learning to administer treatments and manage side effects.
  • Conduct education and outreach sessions on prevention and wellness.
  • Maintain clinical documentation and participate in chart reviews and quality audits.
  • Collaborate with interprofessional care teams and community partners.
  • Provide telephone support and on‑call coverage as required.
Qualifications
  • Master’s degree with Primary Health Care Nurse Practitioner certification.
  • Active registration with the College of Nurses of Ontario (Extended Class).
  • 3–5 years’ experience as an NP in community, primary care, or harm reduction settings.
  • Strong understanding of health promotion and public health principles.
  • Member of NPAO with professional liability coverage.
  • Valid driver’s license and vehicle access required.
