Working with the individuals onsite at Canadian Shield Health Care Service Integrated Care Clinic, the Nurse Practitioner plans, provides, supervises, and ensures direct care for individuals in accordance with the plan of care and established policies and procedures.

The Nurse Practitioner is responsible for being part of the day-to-day operation of the medical services team as well as the larger inter-professional team.

Qualifications
  • Minimum of three (3) years of combined experience in Primary Care settings.
  • Recent relevant experience in assessing clients with complex medical needs is required.
  • Experience in the development, implementation and evaluation of relevant in Nurse Practitioner practices is required.
  • Experience in the development of strategies to build capacity and improve health outcomes is required.
  • Experience in the development of clinical guidelines and protocols, promoting the use of research and evidence-based practice to guide patient-centered care is required.
  • Current Nurse Practitioner Registration with the College of Nurses of Ontario in the Extended Class
  • Preference with experience in geriatrics, mental health, and adult chronic disease management.
  • Current CPR Certification
  • Current Vulnerable Sector Check
  • Bilingualism is considered a definite asset.

Duties and Responsibilities
  • Perform Comprehensive Assessments and consultations for individuals with complex and challenging health care needs.
  • Work in partnership with the leadership team and direct patient care staff to develop, implement and evaluate best practices in clinical care.
  • Provide medical liaison with local hospital(s) and other appropriate health care organizations or facilities.
  • Cooperate with, and/or be available during Ministry compliance reviews pertaining to medical issues.
  • Appropriate assessment and management of controlled substance prescribing within the clinical context.
  • Participate in the identification of clinical or operational performance improvement opportunities and assist in performance improvement activities.
  • Order and interpret diagnostic and laboratory tests.
  • Assess patient needs, diagnose patient diseases, formulate, and prescribe medications and treatment plans.
